Service Description

What to expect 1ST STEP: Click "Book Now" and complete the booking form and payment. After booking is confirmed, our civil surgeon will send you an email with next step instructions. The email will have 4 steps you must complete before your appointment day. YOU MUST COMPLETE THE 4 STEPS before your appointment day (even if your appointment is confirmed). The 4 steps you MUST complete are below; Step 1: Our civil surgeon will send you your lab orders via email pdf. You will be required to print this pdf attachment out and take with you to the nearest labcorp or quest diagnostics. This step will be complete when our civil surgeon gets your lab results back from the lab. Step 2: Our civil surgeon will request you send your vaccination records, IDs and passport size pic of you via email. Once you send this, this step will be complete Step3: Our civil surgeon will send you (via email pdf attachment), the most recent edition of the form I-693. You will be instructed on how to fill your part of this form and return to the civil surgeon via email. This step will be complete once you send our civil surgeon your completed part of this form via email. Step 4: Our civil surgeon will suggest which shots (vaccines) you may need based on your age and shot records sent. This step will be complete once you receive all required shots and send proof of each to our civil surgeon 2ND STEP: You must complete the 4 steps above before your appointment or booked date. On your appointment day, you will sign page 2 of the application and your civil surgeon will sign page 5. The signed forms will be scanned, you will be given a copy (hard copy or emailed) and the originals will be sealed in an envelope and given to you. You are done! Note before you pay, you will be prompted to read and agree to our cancellation and refund policy regarding payments. Labs Your payment covers the labs. The LAB will bill Pedpost Foundation's account on your behalf. Vaccines Your payment does not cover vaccines. Most adults need one or all of these shots, Tdap, hepatitis B, Flu shot, Covid 19 shot, MMR, Varicella, PCV. Our civil surgeon can only tell you the specific shots you need after he reviews your shot records.


Cancellation Policy

Before services begin or are rendered, if we made an error or if an incorrect fee was paid or collected through our payment links, a refund may be issued. Once services or plan benefits have begun, all fees and charges (partial or full) paid are non-refundable. This includes but is not limited to the following: 1. Service fees for ongoing services provided on a retainer or subscription basis. 2. Membership fees for access to exclusive content, resources, or community. 3. Plan fees for access to a specific set of benefits or features for a defined period of time. 4. USCIS Physicals and all paid services we render or plans we offer 5. Partial or full payments Services or plan benefits have begun when our civil surgeon sends you the USCIS Instruction email (regardless of whether the applicant has received or read the email or not). Plan benefits have begun once the prospective member or subscriber's patient portal have been set up. We understand that circumstances may change, but due to the nature of our services and plans, once the benefits have been accessed or utilized, we are unable to offer refunds. This policy applies regardless of the reason for cancellation or termination of the services or plan. We encourage our customers to carefully review all terms and conditions before making a payment and to reach out to our customer support team if there are any questions or concerns regarding our refund policy. Our team is committed to providing additional information and clarifications to ensure that our customers make informed decisions before committing to our services and plans. By engaging with our services or plans and making a payment, you acknowledge and agree to this no refund policy. When using the multi booking feature, booking, cancelation and refund policies may change depending on the service or plan you booked and paid for Additional fees There is a $75 fee for non response or non completion of instructions sent beyond 30 days. There is also an additional $95 fee for positive TB tests that require emedical filing with the state health department. No added fees if you complete your physicals within 30 days and if your TB test is negative.
